Status: copy-ready operator template. This decision record supports inclusion decisions after source verification. It does not mutate package authority, mutate a source registry, approve production use, approve vendors, approve models, approve tools, approve GxP use, or replace owner validation.

## Purpose

Record why a reviewed source can be included, included with caveats, used as perspective-only material, held pending verification, or excluded.
